INITIAL  SPECIALIST/THERAPIST ASSESSMENT  - RECREATION

 Assessment involves:

  • Meeting with the client - discuss recreational goals
  • Liaison with the referrer & any relevant health professionals if appropriate
  • Reviewing and relevant reports or previous written plans
  • Meeting with family members if appropriate
  • Establishing the clients recreational goals, support needs, barriers, strategies etc.
  • Developing a reasonable plan of action with the client
  • Liaison with mainstream or specialized community agencies to establish appropriateness of activity
    (i.e. access, times, transport, informal support options, clients ability to participate in activity, equipment, long term sustainability)
  • Ideally taking the client for a trial of a chosen activity and address any issues which may impact on client’s participation.
    This enables us to establish client’s enjoyment of the activity and wish to participate on a regular basis.
  • Addressing and liaise with funding body regarding paid support needs if appropriate.
  • Providing education for community agency regarding clients specific needs as appropriate
  • Training support workers to ensure program meets client's needs and goals
  • Providing referrer with detailed report outlining actions, outcomes and any further recommendations as appropriate.
This process enables us to get to know the client in a community setting, understand barriers and challenges which may arise and then write a detailed report with reasonable goals for the client.

WHO MIGHT BE REFERRED?

  People with disabilities who:

  •  don't have knowledge or experience in community based recreational activities so require support to explore options to be able to make
    an informed choice
  • have, or need, support workers  for community access/recreation
  • know what they would like to do but don't know how, or have the confidence to get started
  • need specialised equipment to participate in chosen activities
  • are socially isolated
  • transitioning from rehabilitation to the community
  • have an unhealthy focus on therapy, particularly when therapy progress is minimal
  • are not yet ready to explore vocational or study options, participating in chosen recreational pursuits can help develop confidence
    & skills  to move towards work

WHAT WE DO - RECREATION & COMMUNITY ACCESS ASSESSMENT
& SUPPORT SERVICES

  • Support our client to explore, trial and participate in community based recreation options of their choice
  • Work with client and relevant stakeholders to develop  individual client's Recreation Plan based on client goals
  • Provide information on appropriate leisure options considering abilities, challenges and barriers
  • Explore appropriate leisure/sporting options,  trial a previous or new recreational activity and then develop a action plan
    to support the client to participate regularly
  • Provide information on adapted equipment & techniques 
  • Link in with mainstream community based services or sporting groups
  • Refer to specialised recreation and social groups
  • Link clients together if appropaite.  
  • Develop new social programs if appropaite
  • Provide leisure counselling & Resourcing
  • Assessment, establishment & review of leisure focussed attendant care programs
  • Review support needs and manage attendant care programs to best meet people's recreational needs

  • Assess needs & Support to set up holiday/respite options
  • Explore Volunteer Work
